The National Privacy Principles outlined in the Privacy Act set the standard for how private sector organisations are to handle personal information. Healthrise is committed to complying with this standard. As part of our commitment, this Privacy Policy Statement will outline how Healthrise manages the personal information it holds about its clients and their staff.
Openness
When collecting personal information, Healthrise will, where possible, state the purpose for its use, to whom it may be disclosed and how you can request access to the information.
Purpose for Collecting Information
Healthrise only collects information it needs. Healthrise is in the business of providing health management services to individuals, corporations and their staff. Its services include and are not limited to:
- - provision of onsite corporate health clubs & related services
- - health promotion activities & events
- - health screenings
- - corporate health auditing
To effectively provide such services Healthrise need to collect certain personal information to ensure the health and safety of individuals partaking in its services and to assist in determining the type of services it should be providing for a particular client.
Type of Information Requested
Personal information Healthrise may request includes and is not limited to:-
- - name, address, phone number(s)
- - date of birth
- - health information
- - preference for particular activities and/or events
- - feedback on services
Healthrise attempt to collect all information directly from you. It may also obtain information from other sources from time to time.
If you choose not to provide particular information requested that Healthrise require to provide its services effectively, it may not be able to provide the service(s) as intended.
Use of Personal Information
Healthrise uses the collected information to be able to effectively provide its intended service(s). This includes and is not limited to:-
- - effective exercise prescription
- - ongoing health monitoring
- - selection of various activities and services to offer clients and their staff
- - identify health risk factors for individuals
- - identify health risk areas for companies
- - satisfy client needs and expectations
- - assess client satisfaction via feedback
- - provide various information pertaining to Healthrise and its services.
Disclosure
To be able to effectively provide our service(s), Healthrise may disclose information to (and not limited to):-
- - other areas within Healthrise
- - contractors
- - where required by law to relevant authorized bodies
Where Healthrise contract services to third parties to provide services on its behalf, Healthrise will take reasonable steps to protect the privacy of all information disclosed. Where possible Healthrise request such contractors to comply with the National Privacy Principles and Healthrise's Privacy Policy Statement.
Data Quality
Healthrise will take reasonable steps to ensure that personal information it collects, houses, uses and discloses is accurate and correct. It does so via its own internal quality system and auditing procedures.
Healthrise ask that you contact them if you believe any information it houses is incorrect so as it can make the necessary changes.
Personal Information Storage and Security
Personal information collected is stored in a combination of secure electronic and hard copy based files. All reasonable steps are taken to ensure the protection of personal information from misuse, loss and unauthorised access. Such steps include and are not limited to :-
- - secure lockable filing cabinets
- - lockable storeroom / offices
- - limited access via keys and/or security cards
- - passwords & firewalls
- - virus protection
- - quality system procedures
Information is kept for as long as deemed appropriate to be able to provide the intended service or by law. Once the information is deemed 'no longer required' Healthrise will de-identify personal information and/or securely destroy information.
Information Access
Healthrise may be contacted to request a copy of personal information housed. Based on the nature of the request, we may request that relevant forms be completed and in certain cases - as permitted by law - a charge may apply. You will be informed of this at the time of your request.
Complaints
If you have a complaint based on a breach of Healthrise's Privacy Statement and/or the National Privacy Principles you may contact Healthrise head office on Ph: 1300 467 599 to register your complaint.
All complaints will be investigated and you will be contacted in due course on the outcome.
